You need a calorie surplus to put on muscle mass unless you are new to train or not natural. I would recommend 4 exercises and 4 sets each for chest personally

I think 16 sets for chest is overkill unless you specifically do a high volume workout, I'd say 7-12 is sufficient, just depending on your workout style, rep range etc.

Maybe you don't work hard enough. You can check out my chest workout routine and compare. I used to have similar problems, my chest development was lagging behind all other muscle groups.

If im going heavy, then 16+ sets just would not be possible. When I am going heavy I tend to stick with 12 hard sets in the 5-8 rep range and it seems to have worked great for the last few months for me.

But I guess all individuals will react differently, if your chest isn't growing try eating more and maybe training the chest twice a week? If it still isn't growing make sure you increase the intensity of your workouts.

Here is my chest workout, and I see solid results:

Incline Dumbbell Press: 4X6-12
Flat Dumbbell Press: 3X8-12
SS WITH
Flat Dumbbell Flye: 3X8-12
Standing Cable Flye: 7X8-12 (FST-7 Style)

Only takes about a half hour, but hits the pecs hard.
